The Effect of Preoperative Albumin Value on Renal Functions in Open Heart Surgery

Introduction: Low albumin in open heart surgery is an important risk factor that increases mortality and morbidity in the postoperative period. In some patients who are planned for open heart surgery, low albumin levels may be observed in the preoperative or postoperative period due to cardiopulmonary bypass (CPB). Hypoalbuminemia is accepted as a predictive marker for acute kidney injury.
Objective: In this study, it was aimed to investigate the effect of preoperative albumin value on renal functions in CPB-guided cardiac surgery.
Method: In this retrospective study, patients were divided into three groups according to their preoperative serum albumin levels. Patients with consecutive CPB-guided isolated coronary artery bypass graft surgery with additional disease or untreated serum albumin level ≥3.5 mg/dL Group 1; Between 2.5-3.5 mg/dL Group 2; Patients with ≤ 2.5 were named Group 3. Statistical analyzes were performed.
Results: Gender, age, body surface area, flow, ejection fraction percentages, coronary artery bypass graft numbers, cross-clamp times and total perfusion times were similar between the groups (p>0.05). There was a significant difference in renal function injury classification of the groups in KDIGO-I and KDIGO-II (p<0.05), but KDIGO-III, extubation time, length of stay in the intensive care unit, length of hospital stay and mortality rates were similar (p>0.05). ).
Conclusion: According to the KDIGO guideline for damage classification of renal functions, we think that renal damage increases with the decrease in preoperative serum albumin level.
